CLINICAL MEDICINE PROGRAM

 

The education program consists of 72 weeks of clinical training. Every student takes 48 weeks of core clinical rotations in the five major specialty areas – 12 weeks of internal medicine, 12 weeks of surgery, and six weeks each of obstetrics/gynecology, pediatrics, and psychiatry. In addition to the core rotations, all students must complete four weeks of primary care, a four week medicine sub-internship, a four week medicine selective and a four week pediatric sub-internship or selective. To complete the clinical requirements, every student takes 24 weeks of electives.

INTERNAL MEDICINE:

The Internal Medicine Clinical Clerkship is a required, 12-week rotation performed by the student at one of several clerkship sites in the United States and United Kingdom.  This course is designed to develop the students’ logical approach to the diagnosis and treatment of the patient’s complaint. This involves obtaining a complete history, eliciting and assessing information from the patient, performing a competent physical examination, and formulating a differential diagnosis list in order of probability with a diagnostic and management plan.

The overall objective of the third-year clerkship in Internal Medicine is to ensure that every student develops a systematic and effective approach to clinical problem solving in adult patients.  Each student will obtain a precise, thorough, and reliable medical history and perform a complete and accurate physical examination on two to three new patients every week.  The student is expected to integrate medical facts and clinical data, weigh alternatives, and understand the limits of knowledge and incorporate risk and benefit analysis in the care of the patients they are following.  Finally, the student is expected to demonstrate integrity, compassion and respect for patients and other medical staff. ²

 

FAMILY MEDICINE:

The Family Medicine Clinical Clerkship is a required 6-week rotation performed by the student at one of several clerkship sites in the United States or United Kingdom.  This 6-week clerkship allows students to work one-on-one with family physicians in high volume family practice settings.  Direct observation of student performance by faculty will provide a major portion of data needed to asses students knowledge and skills.  Students also document their patient care experience using logbooks.  Students prepare a family case study paper that assesses their ability to integrate and communicate the content of the clerkship experience. ²

 

PEDIATRICS:

The Pediatrics Clinical Clerkship is a required 6-week rotation performed by the student at one of several clerkship sites in the United States or United Kingdom.  This 6-week clerkship allows students to gain clinical experience in evaluating children (both sick & well) through history taking, physical examination and laboratory data.  Special emphasis is placed on: growth and development; nutrition; fluid and electrolyte management; common infections; and preventative Pediatrics. ²

 

PSYCHIATRY:

The Psychiatry Clinical Clerkship is a required 6-week rotation performed by the student at one of several clerkship sites in the United States or United Kingdom.  The Psychiatry Clerkship will introduce students to psychiatric patient, diagnosis, and treatments in a clinical setting.  Emphasis will be placed on disorders likely to be encountered by primary care physicians, and on treatments appropriately applied in that context.  Students will participate in psychiatric consultations. A wide range of problems will be encountered. Assessment and explanation of patient’s reactions to medical care will be discussed. Students will assess, write-up, follow inpatients, and discuss recommendations with the patients’ primary services. ²

 

OBGYN:

The Obstetrics /Gynecology Clinical Clerkship is a required 6-week rotation performed by the student at one of several clerkship sites in the United States or United Kingdom.  This 6-week clerkship experience is designed to provide the student with skills and information required for the diagnosis and management of processes and disease states related to the female reproductive system.  It is also directed at how particular physiologic processes in the female may alter the presentation and manifestations of various other medical and surgical problems. ²

 

GENERAL SURGERY:

The Surgery Clinical Clerkship is a required 12-week rotation performed by the student at one of several clerkship sites in the United States or United Kingdom.  The Surgery Clerkship is designed to provide each student with the knowledge base and clinical skills in General Surgery and the surgical specialties necessary for any physician, regardless of ultimate career choice.  This course is designed to develop the student’s skills in: primary patient care, evaluation, therapy, assessment, and resuscitation of medical and surgical patients. There will be special emphasis on establishing priorities of treatment and assessing changes in priorities of treatment, dependent upon the condition of the patient. ²



ELECTIVE CLINICAL ROTATIONS

 

Numerous varied electives are offered at the University’s affiliated hospitals. Additional electives are available at unaffiliated hospitals, but these are subject to the review and the approval of the Dean, School of Medicine. The student who seeks licensure in the US should carefully note that the licensing boards of some states require that students take electives only at affiliated hospitals. This may also be true in other countries. Some states require that each clerkship (whether core or elective) be completed at a hospital with an ACGME-approved residency in that specialty. Since licensing regulations may vary from state to state and from one year to the next, this matter must be considered as the student devises an elective program. Each elective is usually at least four weeks long, and electives of perhaps less than four weeks, such as ophthalmology and dermatology, require the specific review and written approval of the Dean, School of Medicine.

 

General Strategy- The principal objective of the elective program is to provide the best preparation for the student’s career choice, while coordinating a balanced, yet broad clinical experience. In recognition of the individual plans and needs of each student, choices of both subject matter and course location are made by the student, with advice from supervising clinical teachers and with the approval of the Dean, School of Medicine.
